Advancing system-wide population health management for a large healthcare network

CF were commissioned to deliver a population segmentation and PHM programme for a large hospital group in England that serves a population of c.3m people.

Our approach

CF supports healthcare systems to understand their local population’s burden of disease, healthcare needs and healthcare resource utilisation, to redesign patient pathways – improving care outcomes and optimising resource utilisation.

We have supported population health management delivery through this approach in over 50 healthcare systems globally, and our methodology has been adopted as an international standard for population segmentation.

We work to collect and cleanse the available data, undertake population segmentation analysis to identify priority patient cohorts and then work with clinical and operational teams to develop care pathways and models that best serve these patient groups, tailor care to their needs whilst eliminating unnecessary healthcare spend.

What was the impact?

We have co-developed best practice care models for priority cohorts, which provide improved access, streamlined pathways and greater coordination of care . These changes are being implemented currently and are forecast to reduce spend by 25% over 5 years.
